Decisions, actions and meeting evidence

A meeting can produce a decision, an assigned action or evidence that someone completed follow-up work or facilitated the discussion. These are separate records. Inviting someone, assigning work, ending a meeting and generating a transcript do not, by themselves, establish that the work was completed.

Availability: the backend implements these contribution records, but this is a draft explanation of their rules, not an accepted screen walkthrough. The inspected WorkOS 06 Final Screens include a session ledger; they do not establish forms for recording decisions, assigning actions, completing follow-up or independently verifying facilitation. The frontend uses sample meeting views and follow-up wording without those connected forms. Use your organization's agreed work-recording process until the supported interface is confirmed. An illustrated Publish or follow-up link is not proof that these records were saved.

Choose the right record

Record Purpose What it does not prove
Decision Record an outcome with its owner, supporting reference, related work and time. That minutes were written, an external approval occurred or every attendee agreed.
Action assignment Record the responsible person and a due time for follow-up. That the action was completed or a project/workflow task was created.
Follow-up completion Record an outcome against an existing meeting action, with supporting evidence. That another work item was automatically closed or a reward was approved.
Facilitation verification Have a different authorized person verify a participant's facilitation. That hosting, attendance or ending a meeting automatically earns recognition.
Attendance fact Record supported arrival/departure times from the meeting system. Productivity, quality, agreement or successful delivery.

The current decision operation records a structured outcome and references; it is not a free-form minutes editor. Its outcomes distinguish accepted, approved, preventive, recorded and rejected decisions. The evidence should explain the actual decision in its owning record. Do not put an entire confidential discussion into a reference field.

Check who can act

The meeting session's saved roles determine authority for these operations:

  • Hosts, co-hosts, presenters and observer/moderators can record decisions and assign actions.
  • An action's assigned owner or one of those authorized facilitators can record its follow-up completion.
  • Hosts, co-hosts and observer/moderators can verify facilitation. The verifier must be a different person from the facilitator.

Assignees and credited facilitators must already be identifiable participants in that session. An anonymous guest invitation does not provide the signed-in user identity these operations require. An administrator or manager title alone does not grant the meeting role.

The session's people and roles are a snapshot. Editing the meeting plan later does not automatically update them. The current contribution checks also do not establish that every credited person attended or that the session is currently live. Keep participation, role authority and actual attendance distinct.

Prepare evidence before recording an outcome

Before using a supported contribution interface, establish:

  1. The correct meeting and occurrence. Repeated meetings can have different participants and outcomes.
  2. The action being taken and the responsible person. A decision credits its recorder; assignment and follow-up credit the action owner; facilitation credits the independently verified person.
  3. The related work and a stable reference to the supporting evidence.
  4. When the recorded work started and finished. A finish before the start is invalid.
  5. A due date/time for an action assignment. For follow-up, confirm the due time supplied with that outcome; the current implementation does not automatically copy the original assignment's deadline.
  6. Whether this is a new fact or a correction to an existing fact. Do not create a new record merely to hide an earlier mistake.

These are preparation requirements, not invented button names or a claim that a complete form exists. A valid reference is not automatic proof that the platform opened the evidence, verified its truth or checked access to the related work. Review the underlying material through its authorized source.

Keep assignment and completion separate

Assigning an action records the owner and timing. Completion is a later fact linked to that specific action. An authorized facilitator may record completion on the owner's behalf; that does not transfer ownership to the recorder.

The current source keeps the assignment and completion records separately. It does not provide a normal meeting contribution register or return the new action's identity in the assignment response. Finding an action for completion, reviewing its evidence and recovering an uncertain save need a connected interface and source lookup. A session ledger or sample agenda is insufficient.

A second ordinary completion of the same action is rejected when an initial completion already exists. Correcting the existing completion is different from submitting a fresh completion. Simultaneous operations and stale forms still require implementation verification.

There is no demonstrated automatic creation or closure of a WorkOS request, case task or project task from these meeting contribution commands. Record the corresponding outcome in its owning work system when your process requires that handoff.

Verify facilitation independently

Choose a verifier with the required meeting role who is different from the person being credited. The credited facilitator must be an existing session participant. Holding the host role or pressing End meeting does not create independent verification.

The verifier records supporting evidence and the relevant work period. The implementation does not require the credited participant to hold a role named Facilitator, and it does not prove actual attendance from this operation alone. A repeated ordinary verification for the same participant is rejected; corrections retain the original evidence history.

When linking a communication as evidence, the author and verifier must be different. The source derives those identities from the action and its credited person. Consequently, the current decision path cannot attach communication proof where the decision recorder would also verify their own contribution. Ask the owner of the supported integration to resolve an unsupported evidence combination; do not substitute another person's identity.

Correct an existing fact

Corrections and retractions reference the latest version of an existing fact of the same kind in that session, together with a reason. The source retains the earlier record and adds a version. A stale version is rejected.

A retraction records corrective evidence; it does not delete the original assignment, completion or verification. It also does not establish that downstream summaries or recognition have already been reconciled. The current source has unresolved safeguards around retaining the same action or credited person through some corrections, so use the supported review process and verify the resulting references.

If a request times out or its result is unclear, ask support to check the original attempt and current evidence before sending a new operation. Repeating with a new operation identity can create another fact. A success-looking message alone is insufficient when the operation's actual result was unsuccessful.

Read the intended session ledger

The inspected design calls this Meeting governance and Session ledger. It shows the selected meeting and dated session rows, with an Open action for a row. When that view is connected:

  1. Confirm the selected meeting and occurrence.
  2. Review the session state and the recording/transcription status separately.
  3. Open the relevant session to check its current details.
  4. Use the recordings and transcripts guide for available files and viewing access.

The exact entry control for this ledger still needs verification. Its current response does not contain decisions, action items, follow-up evidence or facilitation records. Participant counts include the saved roster; they must not be relabelled as people who joined. File counts can include pending, replaced, deleted or multiple ownership records and do not count only usable recordings. A latest availability date is not a fresh permission to open a file.

The illustrated future Scheduled row does not establish automatic recurrence execution. The design also combines historical sessions with draft/validation controls; the connected screen must reconcile those controls with the actual current plan state.

Understand delayed summaries and evidence recovery

A meeting outcome and a summary that uses it are saved by different parts of the platform. A saved decision or completion can therefore require delivery and reconciliation before it appears elsewhere. Its absence from a summary does not, by itself, prove that your original action failed.

Daily completeness checks use UTC, the shared time standard, and wait until a UTC day has ended before that day can be closed. That boundary may differ from midnight in your local time zone. Closing a day is a background evidence check; it does not mean every summary has already refreshed or that every late event has arrived. You can continue checking the original meeting outcome while support traces a delayed summary.

Support can investigate whether the original evidence was saved and reached the receiving system. The recovery mechanism resends exact stored evidence with its original identity; it does not repeat your meeting action, create another completion or award recognition. A correction is a separate reviewed change that retains the earlier evidence history.

When earlier evidence has changed after a day's evidence was recorded as complete, reconciliation can require investigation instead of an automatic resend. Avoid submitting the same work again just to make a summary appear. Give support the original meeting, occurrence, approximate action time and visible result, using an approved channel. Keep confidential evidence in its owning record. This explains background recovery; it does not imply that a replay button or a complete contribution register is available in the meeting screen.

Understand business meaning and recovery

For managers and executives, these facts can support accountable follow-up when the evidence, attribution and resulting work are reviewed. The meeting source does not decide points, awards, bonuses or employee blame. Attendance and assignment are not outcome judgements. A correction or rejected result records unresolved review questions rather than proving fault.

Problem Next step
The expected form or action is absent Confirm whether the contribution interface has been delivered. Follow-up wording in a prototype is not a working control.
The selected person is unavailable or rejected Check that person's identity and role in the correct session snapshot. Do not assume editing the meeting plan updated it.
A completion or verification already exists Review the original evidence and latest version before requesting a correction.
A correction is stale or points to the wrong action Reload the authoritative record through the supported integration and verify the action/person. Preserve the earlier record.
The save outcome is uncertain Ask support to reconcile the original attempt; avoid creating another fact with a new attempt identity.
A ledger count or capture label seems wrong Check current session and file details. Roster size, attendance, capture state and usable files measure different things.
A saved fact is absent from a summary or recognition view Ask support to trace publication and the receiving system. Saving a fact does not prove downstream acceptance or delivery.

Provide the meeting/occurrence, action type, approximate time and visible result through an approved support channel. Keep confidential evidence in its authorized source. See meeting history, meeting updates and verified responses for their separate evidence and delivery boundaries.
